In order to overcome the inequity of access to specialist stroke care, telemedicine systems that have harnessed computer-based technology have been developed to provide remote specialist assessment of patients with realtime clinical evaluation. An electronic database search was performed of MEDLINE, EMBASE and the Cochrane Library, and extensive manual searching of articles was conducted to identify studies investigating the use of telemedicine in managing acute ischemic stroke. A total of 31 observational and three randomized controlled studies involving 18,690 patients met inclusion criteria. Telemedicine was demonstrated to be safe, effective, feasible and acceptable for managing acute ischemic stroke. It was shown to reduce geographical differences, and increase diagnostic accuracy and uptake of thrombolytic treatment. Practice Points

Silent subcortical brain infarction
Introduction: The silent brain lesions detected by MRI were fairly common not only in first-ever stroke but also in normal elderly subjects. Some recent studies show the possible role of silent sub-cortical brain infarction in ischemic stroke. The aim of this study was to evaluate the frequency of silent sub-cortical brain infarction in acute first-ever ischemic stroke. Methods: In this descrip...

Background: Hyperglycemia after acute stroke is a common finding that has been associated with an increased risk of death. For the last several years, it was believed that post-stroke hyperglycemia may worsen brain infarction in animal models. According to previous studies, the anti-inflammatory effect of insulin has a protective role on ischemic tissues. Glucose-insulin-potassium (GIK) infusio...
متن کاملP188: The Role of Cryotherapy in Progression of Brain Stroke
Stroke is a leading cause of mortality and morbidity in developed countries and has increased incidence due to progression of average population age. Pharmalogical and mechanical reperfusion therapy, as primary therapeutic approaches, are only applicable to less than 10% of patients with a 50-70% efficacy. but about 90%of patients are severe restricted to these treatments. Glutamate excitotoxic...
متن کاملO10: Thrombo-Inflammation in Acute Ischemic Stroke
Ischemic stroke has been classified as a merely thrombotic disease, so the main goal of its treatment is the recanalization of the occluded vasculature. However, despite fast restoration of blood circulation, progressive stroke still develops in many patients, which has led to the concept of reperfusion injury. The underlying mechanism is only partly known. Though, it is accepted now, tha...
متن کاملCardiovascular complications and acute ischemic stroke after the treatment with epinephrine in an anaphylactic patient
Anaphylaxis is a serious clinical condition that can affect all ages and many systems (skin, gastrointestinal system, respiratory and cardiovascular systems). There are a few case reports of cardiac and cerebrovascular complications due to the anaphylaxis and/or the treatment with epinephrine. A 69-year-old female patient with known coronary artery disease (CAD), coronary artery bypass graft (C...
